APPOINTMENTS
At the time of scheduling an appointment, please indicate if it is for a yearly physical or for a routine office visit. Each appointment requires a different amount of allotted time. The Physicians try to remain on schedule; however, emergency situations do take
priority.
Hours are by appointment only.
COPAYMENTS
Copayments are expected at the time of your office visit.
PAYMENT METHODS
We accept cash, checks, Visa, Mastercard, Discover, and American Express.
CANCELLATION POLICY
Please cancel or reschedule all appointments at least 24 hours in advance to avoid a cancellation fee.
REFERRALS
It is your responsibility to notify the office of all insurance referral requests at least 48 hours prior to your appointment.
PRESCRIPTIONS
For routine prescription refills, call your pharmacy or
contact our office during regular office at 314-534-8600.
MEDICAL RECORDS REQUESTS
You may obtain a copy of your medical record created by and/or maintained by your Washington University Physicians provider by clicking here.
ACCEPTED INSURANCES
We accept most major insurance carriers. Insurance plans accepted by Grant Medical Clinic can be found on the Washington University Physicians’ Website. Please have your insurance information available when you call to make an appointment and when you come in for your visits.
